Sliding Fee Discount

The Center provides a schedule of discounts to all individuals and families with an annual income at or below 200 percent of the most recent U.S. Department of Health and Human Services Federal Poverty Guidelines. The Center will offer to all eligible patients a sliding fee discount based on income and family size and no other factor. If client chooses to apply for a sliding fee discount, client must provide proof of income. If client does not provide required documentation, client will not be eligible for sliding fee discounts. The definition of income and family size will be based on the established current Federal Poverty Guidelines (FPG).
