Gateway Community Health Center, its clinics, programs, and affiliates (collectively, “Gateway,” “we,” “us,” or “our”) are pleased to provide you with our websites and other digital service platforms that link to this document (“Digital Services”).

The Gateway patient portal, which has its own privacy and security policies.
How Gateway handles your “protected health information” (PHI) under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Our HIPAA Notice of Privacy Practices is available separately and governs PHI.
