Gateway Community Health Center, Inc. is a federally funded 501(c) (3) private, not for profit corporation. Below please find important information on how to access the wide array of medical, dental and preventive services offered at the Center.

Everyone qualifies for services at Gateway Community Health Center. To register for services, a client must provide a personal photo ID, and documentation that he/she has Medicaid/Medicare or private insurance.

The Center provides a schedule of discounts to all individuals and families with an annual income at or below 200 percent of the poverty guidelines. If a client chooses to apply for a discount, he/she must provide proof of income. The Center uses the most recent U.S. Department of Health and Human Services Federal Poverty Guidelines to determine the discount the client will receive based on income and family size. If the client does not bring proper documentation, he/she will be charged 100% of charges for all services rendered. The registration department will issue the client a card that lists each member of the client’s family and indicates the discount category the client falls into.
