Gateway Community Health Center, Inc., is pleased to offer its patients the opportunity to access their own confidential PHI online by using Gateway’s secure PatientPortal, GatewayToMyHealth. Our Patient Portal is a secure, web-based system that allows real time access to certain PHI. Access your information from the comfort and convenience of your home or office. All you need is a computer or smartphone and Internet connection to access your PHI.
If you are currently a patient of the Center and interested in registering for the Portal, please stop by the Registration Department, at any of our four locations, to request a PIN for GatewayToMyHealth. After registering, you will be able to:
- Update your contact information
- View your medical summary
- View your Lab Results
- Request non-urgent appointments
GatewayToMyHealth is a secure online portal for you to access your confidential PHI. Please note that if you share your Patient Portal User ID and Password with another person, this will allow that person to view your confidential PHI. If a patient shares or misplaces his/her username and password, Gateway Community Health Center, Inc. has no responsibility over any breach of PHI.
HOW TO REGISTER
- Stop by the Registration Department to request your PIN
- Log-in to the Patient Portal
- Click on the “Register” link
- Fill in the required fields
- Create User ID and Password
- Choose your Password Recovery questions
- Enter your PIN to verify your identity
- Confirm enrollment
- Log-in to your account to access your PHI.
